Hepatectomy for Hepatolithiasis

Abstract

|
Hepatolithiasis and associated cholangitis result in progressive biliary stricture, hepatic atrophy, hepatic abscess, and ultimately, in irreversible cirrhosis of the liver and cholangiocarcinoma. Since hepatolithiasis recurs even after its
complete removal, the appropriate operative procedure should be aimed at eliminating the causes of hepatolithiasis. Factors responsible for the occurrence of hepatolithiasis include bacterial infection, stricture and dilatation of the bile ducts. Therefore, the procedure should aim at (1) completely removing the stones present, (2) removing the strictured part of the bile duct causing dilatation of the bile duct and stagnation of bile and (3) providing a good drainage of bile to minimize bacterial infection. To resolve these problems, hepatectomy was performed in 62 cases during the past 6 years. We have clinical analysis of these 62 patients retrospectively.
